NHS Somerset has selected IEG4 to implement a comprehensive digital continuing healthcare (CHC) platform.

The company said this is aimed at streamlining their All Age Continuing Care (AACC) process throughout the integrated care system (ICS).

The new platform will be accessible by staff and patients and enable the digitisation and automation of referral forms. It will also include a portal through which users can access immediate updates on progress, whilst accessing administrative and payment tools where needed.

IEG4 said the CHC platform integrates multiple processes into one digital system that maps data fields to meet the required fields in the Patient Level Data Set (PLDS). This enables an ICS to generate an extract of the data required for the PLDS, a monthly reporting requirement set out by NHS England..

Lynette Emsley, associate director of continuing healthcare and care sector services at Somerset NHS, said: “Putting in place the new digital platform for continuing healthcare in Somerset will make it easier for individuals and their family members/representatives to be involved with the CHC assessment process and for patients and staff to readily access the information they need directly.”.
