AbstractNonsuicidal self-injury (NSSI) is one of the major public health problems endangering adolescents. However, the neural mechanisms of NSSI is still unclear. The purpose of this study was to explore regional brain activity and corresponding brain circuits in drug-naïve adolescents with NSSI using amplitude of low-frequency fluctuations (ALFF) combined with functional connectivity (FC) analysis.

Thirty-two drug-naïve adolescents with NSSI (NSSI group) and 29 healthy controls matched for sex, age, and level of education (HC group) were enrolled in this study. ALFF and seed-based FC analyses were used to examine the alterations in regional brain activity and corresponding brain circuits. The correlation between ALFF or FC values of aberrant brain regions and clinical characteristics were detected by Pearson correlation analysis.

The NSSI group showed increased ALFF in the left inferior and middle occipital gyri, lingual gyrus, and fusiform gyrus; additionally, decreased ALFF in the right medial cingulate gyrus, left anterior cingulate gyrus, and left medial superior frontal gyrus compared to those in the HC group. With the left inferior occipital gyrus as seed, the NSSI group showed increased FC between the left inferior occipital gyrus and the bilateral superior parietal gyrus, right inferior parietal angular gyrus, right inferior frontal gyrus of the insular region, and left precuneus relative to that the HC group.

With the left anterior cingulate gyrus as seed, the NSSI group showed increased FC between the left anterior cingulate gyrus and right dorsolateral superior frontal gyrus. IntroductionNonsuicidal self-injury (NSSI) refers to intentional and direct injury to body tissues in a socially unacceptable way without suicidal intention1. NSSI often occurs in the context of psychiatric disorders, such as major depressive disorder (MDD) and borderline personality disorder2. In addition, it can occur also without accompanying psychopathology3.

Self-injury behaviors are usually adopted by adolescents with repetitive self-injury as an emotion regulation strategy to relieve negative emotions such as anxiety, guilt, loneliness, alienation or self-hatred4,5. The most commonly recognized ways of self-injury include cutting and scratching the skin, hitting the body or a wall6.

The lifetime prevalence of NSSI among adolescents has been reported to be 13–17%7, while the rate among adolescents in China has risen to 23%8. Adolescents who repeatedly self-injury are more likely to develop mental disorders or suicidal behavior9. With the rising prevalence of NSSI10, the disorder has been included in the Diagnostic and Statistical Manual of Mental Disorders (5th edition) draft diagnosis (DSM-5).

Although NSSI has attracted attention due to its high incidence and potential risk of suicide, its neural mechanisms remain unclear.In recent years, functional magnetic resonance (fMRI) has played an important role in the diagnosis of mental disorders due to its non-invasiveness and high spatial and temporal resolution11.

Previous studies on NSSI were mostly based on task-fMRI12,13,14,15, which provided a considerable amount of evidence for the exploration of NSSI mechanisms. However, it is difficult to obtain consistent results due to differences in paradigms or experimental schemes used.
