Stroke silently alters millions of lives every year, and India is on the frontline of this growing health challenge. According to the latest World Stroke Organization data, stroke-related deaths are projected to surge by 50% globally between 2020 and 2050, with nearly 87% of fatal strokes occurring in low- and middle-income countries.

India, facing a 51% increase in stroke cases over the last three decades, reported 1.25 million new strokes in 2021 alone. Consultant Interventional Neurologist at King Edward Memorial Hospital, Pune, Dr Anand Alurkar laid bare the scale of the challenge: “We feel that the figure [1.2 million strokes annually] is actually much higher… closer to 1.5 million per year, which is a huge figure by any standards.” Despite the staggering numbers, he pinpointed the crucial barrier to improving outcomes: lack of awareness.

The imperative of early screening and prevention

Successful stroke prevention, emphasised Dr Sukalyan Purkayastha, Head, Department of Neurointervention & Endovascular Surgery, Institute of Neurosciences, Kolkata, depends on integrating early screening into primary healthcare. “Stroke… is to some extent preventable. Early diagnosis and prevention of risk factors can prevent stroke effectively… When a patient or anyone at high risk visits a physician… parameters such as BMI, obesity, smoking habits, blood sugar, blood pressure, and cholesterol should all be monitored.” Heart health also demands vigilance: “Any irregular heartbeat, like atrial fibrillation, can predispose the patient for stroke.”.

Regular monitoring and adjusting treatment can curb stroke risk, but as Dr Alurkar noted, “many patients take medicines without follow-up checks… which is a mistake.” Lifestyle factors are major contributors, requiring urgent attention. “Controlling diabetes is the top priority… managing hypertension is crucial… Smoking has a multiplier effect… The longer you sit, the greater the chance of a heart attack or stroke.

Dr Tariq Matin, Senior Consultant, Interventional Neuroradiology, Artemis, Gurugram, detailed telemedicine’s central role: “We have been connected with many peripheral centres… communicating via phone, WhatsApp… this is telemedicine… The American Stroke Association 2019 guidelines acknowledged telemedicine’s role in acute stroke care.”.

Dr Alurkar acknowledged, “Comprehensive stroke centres and cath labs… have not yet reached smaller towns and rural areas.” Expertise in advanced stroke care, such as mechanical thrombectomy—a crucial clot removal therapy—is even scarcer outside metros..
